Instructions
All applicants must have an account or create one to complete and submit their Final Impact Report. Keep your username and password handy so you can "save a draft" and return to complete your submission. Also, this username and password will continue to be used for future grant applications.
- LOGIN HERE if you already have a username and password.
- CREATE AN ACCOUNT (New users only - recommended User Name is your Agency Name - an email will be sent to you to set up your password - you might have to check your spam folder)
- RESET YOUR PASSWORD if your email has been used previously, but the password is unknown.
- LOGOUT (Be sure to log out once you have saved your draft or submitted your application)
You can save a draft and return later to complete the application.
To submit the application, first "preview" and then "submit." Once submitted, you cannot edit the application. A copy of your application will be emailed to you upon submission.
Incomplete impact reports will not be accepted. Do not leave any blanks; enter N/A if not applicable.
Final Impact Reports are due by the end of the business day on July 15, 2026.
